  1. What should I do prior to my first appointment?

    • Call you insurance to verify your benefits.

    • Complete the New Patient Paperwork by clicking here, or arrive 15 minutes prior to your scheduled appointment

    • Bring with you your insurance cards, ID, and physicians referral if required by your insurance.

    • Where comfortable clothes and supportive shoe wear.

  2. How long are physical therapy sessions?

    • Sessions are generally 45 minutes in duration, and occasionally can last a few minutes longer.

    • We recommend you arrive 5 minutes prior to your scheduled appointment for all follow-up visits.

  3. What should I do if I am running late?

    • Call or text to SPARK45 ASAP and when safe to do so.

    • If it is your first visit, your appointment may have to be rescheduled.

    • If you are more than 15 minutes late, you appointment will be considered cancelled.

  4. What should I do if I need to cancel or reschedule?

    • Call or text to SPARK45 ASAP. We do generally have a waitlisting of client this allow us to take someone off the list and into the office.

    • If it is less than 24 hours prior to your appointment, you are subject to a $30 late cancellation fee. We will do our best to reschedule you.

    • If it is more than 24 hours prior to your appointment, you are not subject to a late cancellation fee. We will do our best to reschedule you.

  5. Is physical therapy painful?

    • At times, Physical Therapy can be uncomfortable to the point of pain, but in general no. We work out best to relieve your pain, and at times we do need to work through discomfort to allow you to improve. If at anytime during your session, it feels very painful always let your therapist know so we can evaluate the pain and see how we should progress.
